Below is the checklist for a new driving school application.
You must submit:

The MVA will review your application and may inspect your school location and vehicles before approval. You may not operate until your license is approved.
Below is the checklist for the renewal application for 2-year licensing. Applications must be completed and returned to the MVA no later than 15 days prior to the date on which your school Wall License will expire. All forms must be completed in full.
