Purpose

The MVA would like to notify the Dealer and Tag and Title Service industry of a few updates to the VR-005. 

Summary

First, a new section has been created for title service agents only. This section has a field for the 6% collection fee as well as the “paperwork received date.”

Second, a new field for the NAIC number has been added to the insurance section of the form.

The most recent version of the VR-005 shows a revision date of 07-26.  Please discard previous versions, as they will not be accepted after August 19, 2026.
