To obtain this plate please submit the following:

• Application for Special Registration Plates (form #VR-164)
• A valid copy of your FCC Amateur Radio Operators License
• Applicable fee
• If applying for a Disability Amateur Radio Operator Plate, an Application for Individual with a Disability (form #VR-210)​. Must be accompanied with above requirements.
  
• A $40 substitute plate fee will also apply​
